Prior to participating in any athletic activity at Hartsville High School, all student-athletes must complete the following:

1. New Physical for the current school year dated after May 1
2. Completed/Signed Parent Permission and Acknowledgment of Risk Form (third page of the physical paperwork)
3. Concussion Education Google Form
4. Sportsware Electronic Medical Record (2-step process)
5. Covid-19 Pre-activity Form
